How can a PIN request an evoucher

Please note, You will need a document with your name and address on it to prove your identify.  You will be asked to attach it to the form.

1	Click here and enter your email to get verified.
2	Go to your email and find the email from pifvouchers@outlook.com. Click on the verification link in the email to verify your email address/account and you’ll be taken to the request form.
3	Fill in the form, making sure you complete all the ‘starred’ questions and attach the evidence that has your name and address on it.
4	Click ‘Next’ OR ‘Save and resume for later’. NOTE: you won’t be able to move to the next page if any * questions have not been answered.
5	The next page will allow you to check your form and go back if you notice an error.
6	Complete CAPTCHA and click ‘SUBMIT’
7	You will get a notification screen with a verification code. Text this to 07429 681278
8	The form will come to me to be processed.
	Please be aware that I work 4 full days a week and run this organisation on my own in my spare time(evenings and weekends). Please allow 1 working day as a rule before receiving a voucher. Please say if it’s urgent and I’ll see what I can do
	If I’m happy with the form then I will send you the voucher. Please keep an eye out for a text from me telling you it’s on the way and asking for feedback.
	If you do get a voucher, PLEASE REMEMBER TO SEND ME THE RECEIPT.
